The forms are served from the Department’s current site at hindudept.gov.lk while they are being moved onto this one, so a form opens on that address rather than this one.

Submitting a completed form
Bring the form in person during office hours, or post it to the Department. Where a form concerns a temple, an Aranery school or an institution already on the register, write its registration number on the form — it is the fastest way for an officer to find the file.
